How does the Ark Complex Care service work?
When we provide intensive or long-term care for people in their own homes, we’ll create an individual Complex Care plan to look after them.
This is how it works:
- a dedicated case manager will attend all reviews and assessments and give your client a single point of contact
- specialist intervention and assessments when they need it
- care workers and healthcare professionals to match their needs
- 24 hour on-call support from case managers and healthcare professionals
- a Complex Care package at short notice
- an efficient rota system.
All our case managers are experienced in NHS continuing care and currently work for primary care trusts in the UK.
You’ll get an individual Complex Care plan
Once we’ve received your referral, we’ll visit your client at home or in hospital and talk to you both about the type of care they need. We’ll develop a care plan and assess the risks with the help of the client, carers, family members and healthcare professionals. The care plan will look at the health, social care and level of support your client needs. Once we’ve agreed this with the relevant funding authority, we’ll create a client contract to outline roles and responsibilities. Your client will receive:
- Complex Care information sheet
- contract terms and conditions
- care plan and diary
- care assessments
- risk assessments
- care workers’ time sheets
- emergency contact sheet
- complaints and compliments slip with a prepaid envelope.
Regular visits
